Eligibility Requirements:

To be eligible for benefits, you must work a minimum of 24 hours and be classified as benefit eligible.

Medical –Harvard Pilgrim Healthcare

4 Options Available –
PPO & HMO, HMO Focus, High Deductible Health Plan (HDHP)

Individual and Family plans available

HMO & PPO – $2000(Ind.)/$4000(Family) deductible – Employee pays first $1,000.00 (Ind.)/$2,000.00 (Family) HRA plan to cover deductibles – Agency pays deductible after employee pays their portion

HMO with Focus Network – Lower cost than traditional HMO, *must make sure all providers are part of focus network.  $2000(Ind.)/$4000(Family) deductible – employee pays first $1000(Ind.)/$2000(Family) – HRA plan to cover deductibles – Agency pays deductible after employee pays their portion

HDHP – $3000(Ind.)/$6000(Family) deductible – Employee pays 100%

Dental Insurance – Delta Dental

Individual and Family plans available
$1000 coverage per calendar year/per person

Vision Insurance – Eye Med

Frequency – Exam (every 12 months)/Lenses (every 12 months)/ Frames (every 24 months)
Prescription lenses (single, bifocal, and trifocal) $25.00 copay
Frames ($130 allowance), 80% of charge over $130
Disposable contact lenses ($130 allowance), any balance over $130

Life Insurance – Hartford Life

Employees can purchase up to 5X salary

Short Term Disability – Hartford Life

7-Day waiting period then pays 70% of gross salary up to 3 months

Long Term Disability – Hartford Life

Pays 60% of gross salary – Must be out for 90+days

Retirement Plan 403(b) – Fidelity

All employees are eligible – up to a 4% match for any hourly staff working 20+ hrs. 100% vested after 5 years of service

Flexible Spending Plan (Section 125 Plan) – Pre-tax benefit

Medical – $2600 max/year ~ Medical FSA includes $500 Rollover Option

Accrual rates are based on job classification, hours worked and years of service.  Accrual starts from date of hire and is available to use after 90 days
