Advanced Illness Management

The Advanced Illness Management Program includes comprehensive medical interventions, teaching, monitoring and support for patients with chronic illnesses such as COPD, CHF, diabetes, cancer and other complex illnesses. Palliative care, a key component of the program, offers symptom management, emotional support and care coordination to improve function and quality of life for patients with life-limiting illnesses. Goals of care conversations are used to align the care provided to the needs and wishes of the patient.
