Great opportunity for a Registered Nurse to work alongside the clinical management team.

The Clinical Coordinator reports to and functions under the general direction of the Director of Clinical Services, performs a variety of duties in the coordination of the delivery of care to patients and families in their homes.  The Clinical Coordinator carries out their responsibilities consistent with Agency policies, practices, mission and vision, and with instructions as set forth by the Director of Clinical Services.
As a key member of the health care team, the Clinical Coordinator must interact courteously and effectively with patients and their families as well as with co-workers from all Agency departments, community resources, and with patients' physicians in order to facilitate safe and efficient patient care.

Position Summary
Works closely with clinical staff to provide necessary services for patients and families in a timely manner.
Provides resources and support to staff.

Demonstrates expertise and knowledge in a variety of areas, including, but not limited to

          a.  Current methods of care and treatment
          b.  Policies, procedures and protocols which are specific to the Agency.
          c.  The Agency Infection Control Plan.
          d.  A working knowledge of all reimbursement criteria.
          e.  A working knowledge of all regulations.

Provides coordination of patient services across agency departments

Demonstrates a caring and considerate response to staff needs. 

Assures that regulatory and licensing requirements for the provision of care are met.

Has a working knowledge of the Medicare Conditions of Participation.

Assists clinical managers in assuring quality care for patients.

Assists the clinical staff with patient triage, patient resources, and documentation

Assists with orientation and preceptor of clinical staff.
Assists with orientation and supervision of the Home health aides services for patients
Assist in establishing plans for care delivery, including case conferencing and referral to the Ethics Committee.
Ensures patient services are skilled and provided at the appropriate frequency identified by the clinical assessment identifying patient needs.
Monitors staff assignments, caseloads, and team productivity.  Promotes efficient utilization of resources to achieve desired clinical outcomes and within episode reimbursement.  Performs home supervisory visit
Develops and maintains a system of patient tracking for the patient’s clinical condition.  Refers and discusses high utilization and problem prone cases with Director of Clinical Services.
Facilitates appropriate communication and documentation between patient, team members, medical professionals, facility personnel and referral sources.
Conducts individual caseload reviews on a regularly scheduled basis according to the agency’s standards. Case conferences with staff as needed to meet the needs of challenging clients.

Assures communication and coordination of services on the part of each team member to determine the adequacy of the plan of treatment and appropriateness of continuation of care.
Works with Quality Management, clinical staff and medical records personnel to ensure accurate and  timely paper-flow, as well as, completion of documentation in the electronic record.
Contributes to Performance Improvement Activities
Contributes to performance improvement efforts., assisting with the development of written plans, policies   and procedures.
Demonstrates knowledge of risk management activities; completes variance reports accurately, appropriately, and in a timely manner.
Participates in activities related to staff performance.  Monitors trends. 

Provides leadership in promoting and maintaining the objectives of the clinical programs.
Maintains an awareness of individual competencies.  Encourages each individual to achieve and use  achieved  competencies.
Encourages and assists personnel to continue self-development and professional growth. 

RN license in Massachusetts and New Hampshire
Exceptional interpersonal skills
Strong organizational skills and customer service
Good problem solving and analytical skills are required
Must have computer skills with experience in multiple systems (Microsoft Outlook, Excel, Word, Cerner, Homeworks, Roadnotes)
