A key nursing position as a critical team member in managing and delivering home care nursing to patients in their homes. Medical surgical nursing within the home health practice setting as well as hospice and end of life nursing within the palliative care and hospice practice setting is the focus of this position. Responsible for providing nursing care to patients and education to families in their homes; coordinates activities with other community and Agency staff and carries out special activities on assignment.
As a key member of the health care team, the Evening Home Care Nurse will interact with other members of the agency as well as providers across the continuum. Strong nursing assessment and care planning skills, critical thinking and a focus on patient experience and safety are key.
Required Education and Experience
Graduate of an accredited school of nursing, baccalaureate degree in nursing strongly preferred.
Registration and current RN Licensure in the state of Massachusetts and New Hampshire in good standing required.
Professional certification highly encouraged.
This is a full-time position, and days and hours of work are Monday through Friday, 3:00 p.m. through 11:30 p.m. This position will also be required to work rotating weekend shifts every other weekend. Days and hours of work are based on operational needs with some flexibility based on position.
