Description

Direct, coordinate and evaluate the delivery of Telemonitoring and Televideo services. Ensures compliance and consistency with agency policy, State and federal home health regulations. Responsible for the management of cost effective, efficient care to Telemonitoring patients ensuring the delivery of outcome based care and value to meet the individual needs of patients.  Acts as a role model and mentor professionally and clinically for the implementation of the organization’s Mission and Core Value statements. Key member of the team to grow the service line.

As a key member of the health care team, the Clinical Manager must interact courteously and effectively with patients and their families as well as with co-workers from all Agency departments, community resources, and with patients' physicians in order to facilitate safe and efficient patient care.

Required Education and Experience

Current professional registered nursing (RN) licensure in the states of Massachusetts and New Hampshire

BSN from accredited nursing program.

Valid driver’s license, auto insurance, and current CPR certification.

Current Immunizations per agency requirement (i.e., TB test, Hepatitis, etc.)

3 years’ experience in Med/Surgical nursing or home care experience. Supervisory experience preferred.

Ability to read/write and communicate in English.

Excellent interpersonal skills.

Ability to function well in very busy situations.

Demonstrated strong organizational, planning, problem solving, decision-making, leadership, and motivational skills.

Demonstrated ability to communicate effectively in writing and verbally with people of varied backgrounds and levels of understanding.

Previous experience in directing and managing a clinical team.

Proficient computer skills and ability to use word processing, database and systems applications.
