The Advanced Care Clinical Coordinator (ACCC) will work to improve transitions of care (from acute and SNF facilities) to home. This position will ensure care is being delivered in the most appropriate setting for individuals identified as high risk for readmission to hospital or ED.  The ACCC will work closely with the hospital/SNF care management teams, hospital staff, PCP, and other Community Partners to monitor utilization of services, identify medical and social drivers of ED and inpatient utilization, and respond appropriately.  The ACCC will work to improve the quality of care by reviewing clinical information and recommending the most appropriate care setting in collaboration with agency and facility clinical staff, patients, families, and physicians.

Required Education and Experience

  • Bachelor's Degree required
  • Registered nurse with an active professional license in Massachusetts and New Hampshire (or willing to obtain both)
  •   3-5 years of clinical experience in home health, post-acute or acute care setting  
  • Ability to read/write and communicate in English.
  • Excellent interpersonal skills.
  • Ability to function well in very busy situations.
