The MCH Nurse provides nursing services to Maternal/Child Health patients via a primary care nurse model under the guidance of the MCH Program Manager. 

Essential Functions 

  • Assess and implement physical and psychosocial needs of the patient following established Standards of Nursing Practice and VNA procedures. 

  • Assess and implement patient/family learning needs, style and limitations and makes adjustments for delivery of information. 

  • Identify abuse and/or neglect and identifies reporting mechanism per policy. 

  • Involves the patient in understanding and become involved in the course of treatment. 

  • Collaborates with patient /family and other health care providers and/or community resources with planning of care and discharge. 

  • Develops a realistic written plan of care toward measurable goals. 

  • Develops and evaluates the plan of care in partnership with the patient, representative (if any), and caregivers. 

  • Provides ongoing interdisciplinary assessment of the patient. 

  • Plans death at home when appropriate with patient/family and other members of the health care team. 

  • Provides for reassessment and evaluation of those plans of care at regular intervals. 

  • Assists in establishing daily work plan based upon patient/family priorities of service and total area needs. 

  • Medication reconciliation with patient and physician consistently. 

  • Continuously assesses/evaluates patient and family progress and goal achievement, keeping Clinical Manager advised and justifying continuation and/or termination of services. 

  • Utilize equipment according to policies and procedures. 

  • Educating patients of safe/effective use of equipment provided by vendors. 

Position Type/Expected Hours of Work & Travel

This is a part-time position, and days and hours of work are Monday through Friday, 8:00 a.m. to 4:30 p.m. This position will be expected to cover weekends and holidays on a rotating schedule. Days and hours of work are based on operational needs with some flexibility based on position.  

Qualifications

  • Registration and current RN Licensure in the state of Massachusetts and New Hampshire in good standing required. 

  • Pediatric Oncology experience required and/or completion of 2 required oncology training

  • CPR certification required unless ACLS certified. 

  • Minimum of one year of acute medical/surgical nursing experience required.
