Responsible for the clinical provision of care and the patient/family experience within the home health certified program. Key leader in the development and building of clinical teams and clinical expertise across assigned branches and in collaboration with other clinical directors. A professional skilled in clinical care delivery and management of clinical programs who utilizes clinical practice guidelines, regulations and innovation in ensuring the highest quality of care and patient experience.  Identifies opportunities and needs within the organization as well as partnering with Wellforce system leaders to support the overarching goals of care in the home. 

In concert with governing, regulatory and accrediting bodies, and in collaboration with managers, physicians and clinical leaders, ensures safety and quality standards of practice, fosters cooperative working relationships, and strengthens the teams care delivery.

Ensures that the agency is able to plan, implement and evaluate progressive, innovative services, programs and activities and meet the standards to be compliant with all regulations and standards.


  • Registered Nurse  licensed professional, Masters level degree or above in clinical, health care administration or community health required.  Clinical licensure in both the Commonwealth of Massachusetts and the State of New Hampshire required.
  • Minimum of 5 years in progressive leadership within home care or acute care setting with strong emphasis on home care experience.
  • Minimum 5 years clinical experience in home care or other community based setting.
  • Must demonstrate ability to speak confidently and present with strong knowledge base in a confident manner.
  • Strong critical thinking skills and ability to assess and synthesize data for planning and assessment of outcomes.
  • Possesses proactive thought leadership skills and uses appreciative inquiry within the leader role.
  • Ability to write and develop materials for target populations.
  • Ability to read/write and communicate in English. Multilingual a plus.
