Job Description

A Registered Nurse who provides assistance to the hospice teams by gathering and entering all data for a hospice admit.  Prioritizes admits for admission nurses. Maintains and promotes optimal relationships with referral sources. Ensures that all referrals are handled efficiently and effectively. Provides customer service to all referral sources and families interested in referring to Merrimack Valley Hospice. Serves as a resource to discuss regulations, determination of hospice eligibility, and appropriateness for hospice.  Provides resource information if patient/client is not eligible or not appropriate for hospice.  The Hospice Referral Nurse gathers necessary information and uses clinical expertise to manage timely and accurate referral processing and works closely with the clinical department to coordinate patient care. The Hospice Referral Nurse also evaluates referrals remotely, if needed, and orchestrates transfers onto Hospice services.

Essential Functions

  • Responds to all referrals and inquiries promptly.
  • Assigns referrals to Admission Nurse in an efficient manner.
  • Ensures that desk/phone are covered during working hours.
  • Notifies the appropriate clinical team of impending admission and any pertinent information.
  • Obtains History & Physical for each referral.
  • Develops and maintains effective relationships with referral sources.
  • Works with families to allow visits within prescribed guidelines by Admission Team.
  • Reviews reports daily and ensures follow-up activities are completed on previous informational visits and pending patients.
  • Converts pending referrals to admission.
  • Works in a collaborative manner with VNA to increase referrals.
  • Orients and educates team to admission process.
  • Develop and maintain accurate tools for team to admit patients efficiently and per individual needs of hospitals and facilities.
  • Tracks all referrals and related information.
  • Aggregates performance data including but not limited to conversion rate, NTFC classifications, and time of referral to admission.
  • Completes remote evaluations on patients as needed
  • Assists in coordinating discharge of patients from hospitals and other facilities, onto our services
  • Collaborates with HPH staff on referrals for the hospice house

Position Type/Expected Hours of Work & Travel

This is a full-time position, and days and hours of work are Monday through Friday, 8:30 a.m. to 5 p.m. with some flexibility based on position.

Qualifications

Required Education and Experience

  • Registered Nursing license in MA and NH
  • Minimum 2 years of relevant clinical experience; homecare and hospice case management experience desired.
