Job Description
Up to $5,000 Sign-on Bonus available!
The Hospice Medical Social Worker will provide direct service to patients and families.
Responsibilities:
Provide psychosocial and financial-environmental assessments related to patient's illness, needs, response to treatment, the patient's financial resources, and availability of community resources.
Develop a care plan and coordinate this plan in collaboration with the physician and nurse or therapist.
Evaluate the impact of the care provided
Provide counseling services to patients to assist the patient to cope with his/her medical condition and to respond to treatment
Provide short-term therapy with goal-oriented interventions directed toward management of terminal illness, reaction and adjustments to an illness, and resolving conflicts related to the chronicity of an illness
Provide Community Resource Planning by establishing the need for and making referrals to community resources such as supplemental health care, meals on wheels, food stamps, advocacy and linkages
Provide long-range planning by assessing the patient's needs for long-term care, including evaluating the home and family situation, helping the patient and family to develop an in-home care plan, exploring alternatives to in-home care and arranging for placement.
Consult with other team providers to assist them in understanding significant social and emotional factors related to the patient's health problems
Position Type:
This is a full-time, day position
Qualifications
Master’s degree in social work from a school accredited or approved by the Council on Social Work Education.
At least one year of social work experience in a health setting.
Current licensure in Maine and New Hampshire
