The Telehealth Staff Nurse coordinates and provides telehealth monitoring services.  Provides consultation and collaboration to patients and staff; oversees the Tele-monitoring assessment process of patient’s physical status and clinical recordings, and provides appropriate follow-up with patients, clinicians, and physicians to enhance patient optimal self-care disease management and education with the goal of reducing acute care hospitalization. Responsibilities include triaging patients based upon telehealth data, coordination of patient care needs with the patient’s medical stakeholders including, but not limited to the home health clinical staff, physicians, and additional patient stakeholders as needed,  equipment inventory, education of staff related to telehealth, utilization of telehealth surveys and educational materials in caring for patients, patient education, establishment of telehealth services and promotion of the program internally and externally.

Essential Functions

  • Demonstrates and utilizes evidenced based standards for Telehealth practice.
  • Demonstrates ability to observe, analyze, and interpret general and technical clinical data both verbally and in writing.
  • Works collaboratively with intake, clinical staff, referral sources, and patients to identify those patients who qualify for telehealth services.
  • Utilizes internal databases to identify those patients who qualify for telehealth services.
  • Oversees and/or provides registration for all patients into the Telemonitoring program software; oversees performance of Telemonitoring including establishment of assessment parameters in collaboration with the ordering provider and communicates with key program stakeholders which may include the case manager and physician when changes in the plan of care are required.
  • Utilizing technology, communicates with others, as necessary in an effective and efficient manner.  Responds to inquiries in a timely manner.  Documents appropriately.
  • Works with Telemonitoring team to perfect software platform and problem solve as needed.
  • Communicates with Telehealth Clinical Manager regarding troubleshooting and equipment problems based on individual issues or needs.
  • Collaborates with physicians and other key stakeholders in consultation and caring for patients on Telehealth services.
  • Evaluates data transmitted by patients.  Follows up appropriately which may include, but is not limited to, communication with patient/caregiver, physician, primary clinician, or scheduling a visit.
  • Promotes patient self-management including, but not limited to educating patients and caregivers regarding disease process, signs and symptoms to report, medication management, and lifestyle changes needed relative to disease process.
  • Documents in electronic telehealth record as well as electronic medical record. 
  • Reviews and responds to all telehealth reports in a timely manner.
  • Provides updates and/or reports to the primary care/covering clinician as requested by the clinician.
  • Obtains changes in orders from physicians as needed.
  • Follows up via phone contact with the patient with abnormal results from Telemonitoring to assess for the need for a home visit or to retest.
  • Provides documentation to meet patient needs, legal and reimbursement requirements, and other agency requirements.
  • Participates in IDT meetings to provide Telemonitoring education and support to all clinicians
  • Attends meetings with PHO case management as requested by the Telehealth Clinical Manager or representative.
  • Makes home visits if necessary for installs, clinical assessment, troubleshooting and pick-ups.
  • Ensures compliance within guidelines set forth by regulatory agencies (DPH, ERISA etc.) and demonstrates compliance with Home Health Foundation policies and procedures.
  • Practices confidentiality principles set by the agency and federal HIPAA/HITECH guidelines.

Position Type/Expected Hours of Work & Travel

This full-time position. Days and hours of work are Monday through Friday, 8:00 a.m. through 4:30 p.m. with some flexibility based on position. This position is required to work weekends and coverage for holidays and other Telemonitoring staff as needed.

Qualifications

  • Current professional nursing (RN) licensure in the state of Massachusetts and New Hampshire.
  • Graduate from accredited nursing program.
  • Valid driver’s license, auto insurance, and current CPR certification.
  • Current Immunizations per agency requirement (i.e., TB test, Hepatitis, etc.)
  • 1 year of acute care experience.
  • 3 years’ experience in community or home care.
