Referral by phone
To arrange for care by phone, please contact our Referral Department at 978-552-4444 or 800-333-4799. To ensure that each patient receives prompt attention, we provide:
- Referral services 24 hours a day, 7 days a week
- 24-hour telephone support
Although every patient is different, early referrals to hospice are most beneficial to the patient and family. Early referrals to Tufts Medicine Care at Home can allow the patient and family to focus on the time they have together.

Payment for services
The cost of care is typically covered by health insurance – including most private insurances, Medicare and Medicaid, and Senior HMOs.
Hospice care, including all medical equipment, supplies and medications related to the hospice diagnosis is typically covered by private health insurance including Medicare and Medicaid.
Private care services payment rates are determined by the type, number and frequency of specific services needed and are available upon request.
