Lawrence, MA –  Home Health VNA is pleased to offer a free informational brochure “Know Your Choices, A Guide for Patients with Serious Advancing Illness”.  Produced by the Massachusetts Department of Public Health, the guide is an important resource for all individuals and contains information on advance care planning, palliative care, hospice care and medical orders for life sustaining treatment.  To download your free guide about this regulation and to learn about how you can begin the conversation about advanced care planning with your family and loved ones, please go to   http://www.homehealthfoundation.org/patients-resources or call 978-552-4186.

In Massachusetts, a newly implemented regulation requires that all licensed hospitals, clinics, and long-term care facilities distribute information about advanced care planning to adults with serious illness. No one likes to think or talk about illness and future medical care choices, but when faced with a serious illness, it’s critical that your doctor, family and loved ones know your health care preferences.   The goal of this guide is to help all individuals start the conversation and make their wishes clearly known before a serious illness or accident forces the conversation.

Home Health VNA, the leader in home health care, is a not-for-profit agency affiliated with HomeCare, Inc. and Merrimack Valley Hospice.  Together the three agencies serve more than 90 communities throughout the Merrimack Valley, Northeastern Massachusetts, and Southern New Hampshire.
