Tufts Medicine Care at Home offers comprehensive medical and supportive services designed to maximize health, independence, and quality of life.
Whether patients are at home, in a hospital, a long-term care facility, an assisted living facility, or at High Pointe House, our expert staff offer physical, spiritual and emotional care specific to those nearing the end of life. Bereavement support and counseling are also offered to families and loved ones.

Commonwealth Nursing Services (CNS) is the private home care agency for Tufts Medicine Care at Home, offering dependable and affordable in-home solutions to enhance recovery, ensure safety, and increase independence.

High Pointe House

High Pointe House, the hospice and palliative care residence of Tufts Medicine Care at Home, is located in Haverhill MA. It is a licensed, acute care hospice residence that provides a home-like alternative to hospitalization for patients in need of specialized end-of-life care.

I am writing again to thank you for sending such lovely people out to my home after surgery. To be able to have the same great care that I received after my first hip replacement is a wonderful thing. Sharon is absolutely the best, funny and caring. Going on walks outdoors was the nicest part of all.

I would like to recognize some very important people who nursed me back to health after surgery. My Home Health VNA nurse, and physical and occupational therapists were so quick to start my road to recovery and they stuck with me for 3 months. They were funny, helpful, caring, and made me feel important, as well as comfortable. Each one of them deserves a GOLD star! I am forever thankful and grateful.

High Pointe House is filled with “angels on earth”. From the minute we entered the front door to the day my husband, Ed, passed away, we were treated with the highest level of respect and compassion, allowing me to spend time with Ed as his wife while the nurses and aides cared for his medical needs. We are so grateful for sharing Ed’s last days in a “home” full of love.

Jill has been my Physical Therapist since I came home from rehab in December. She was a godsend to me. After my knee replacement, I never thought I would be able to raise my right leg again.
